Biocompatible Dentistry: How Material Choices Are Matched to the Patient
Biocompatibility is a relationship between a material and a specific person, not a fixed property of the material. Here are the main material categories, how compatibility testing works, and the questions that produce genuinely useful answers.
Two patients can receive an identical filling, placed by the same dentist on the same afternoon, using the same material from the same box. One does well. The material categories to understand
Composite resins
Tooth colored materials that bond directly to the tooth, used for most modern fillings. They avoid mercury entirely. The consideration is resin chemistry, since many formulations are based on Bis-GMA, a bisphenol A derivative. BPA free and Bis-GMA free formulations are available, and a practice should be able to name the specific product it uses.
Ceramics
Porcelain and zirconia restorations avoid both metal and resin in the restoration body, though the cement bonding them into place is its own material worth asking about. Ceramics perform strongly for crowns, inlays, onlays and implants, and are the usual answer for patients seeking a metal free mouth.
Metals
Gold alloys have a long clinical record and are well tolerated by many patients. The concerns in biologic practice center on amalgam's mercury content and on galvanic activity, meaning the small electrical currents that can occur between dissimilar metals sitting in the saliva of the same mouth.
Glass ionomer
A material that bonds chemically to tooth structure and releases fluoride. Useful in specific situations, and its fluoride release means it fits some biologic protocols and not others depending on the practice's position.
How biocompatibility testing works
Biocompatibility testing, sometimes called serum compatibility testing, uses a blood sample to screen a patient's immune reactivity against the chemical components used across dental materials. The laboratory returns a report grouping components and the products containing them into categories from least to most reactive for that individual.
The clinical value is that it converts a general discussion into a specific list. Rather than choosing between material philosophies, the dentist selects from products the patient's own results support.
When testing is most worth doing
- Before extensive restorative work, where one material choice repeats across many teeth
- For patients with autoimmune conditions or chronic inflammation
- Where there is a history of unexplained reactions, including to jewelry or metals
- Before implant placement, which is intended to be permanent
- When replacing amalgam, so the replacement is chosen deliberately
Why the conversation matters as much as the test
Testing produces data, and data still needs interpretation against the clinical demands of each tooth. A material that suits a patient perfectly may not withstand the bite forces on a particular molar, in which case the dentist has to find the best available option that satisfies both requirements. That is a discussion, not a lookup.
